PARTICIPANT WAIVER, RELEASE, AND INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate in the Men's Range Day event (the "Event"), I, the undersigned, acknowledge, appreciate, and agree that:
1. Acknowledgment of Risk: I understand that participating in a shooting event involves inherent risks, both known and unknown, including but not limited to severe physical injury, hearing loss, eye injury, and death from the discharge of firearms.
2. Rules and Safety: I agree to strictly follow all safety instructions, range rules, and commands given by event range safety officers and organizers at all times. I confirm that I am legally permitted to handle firearms and am not prohibited from doing so by any state or federal law.
3. Assumption of Risk: I knowingly and freely assume all such risks, both known and unknown, associated with my participation in this event, including risks caused by the negligence of other participants, organizers, or staff.
4. Release and Waiver: I hereby release, waive, and forever discharge Memorial Baptist Church, its directors, officers, employees, volunteers, and event sponsors from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during my participation.
5. Indemnification: I agree to defend, indemnify, and hold harmless the organizers from any and all claims or lawsuits brought by or on behalf of me, my family, estate, or heirs, arising out of my participation in this event.
6. Photo/Media Release: I hereby grant Memorial Baptist Church the right to photograph, record, and use my image and voice for promotional or marketing materials.
BY ELECTRONICALLY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ THIS WAIVER AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING MY RIGHT TO SUE.
